Joining
Medical forms
We’re required to collect basic medical information for each scout before they take part in pack events. You need parts A and B of the BSA Annual Health and Medical Record.
- Part A is the important one — informed consent and general information.
- Part B is health history, intended for situations where a non-parent might need it, such as camping without you present. Because we expect a parent at every activity we don’t actually use Part B, but we’re still required to collect it.
Please don’t email these to us
Completed medical forms should be handed over on paper, for HIPAA reasons. Bring them to the next meeting. The blank forms live at scouting.org/health-and-safety/ahmr.
Everything else
Some events need extra paperwork — the den or pack leaders organizing it will send it out ahead of time.
